Access Point Levels Description

  • Level 1- An agency that completes the Mo BoS CoC Prescreen tool and assists household or individuals in getting access to services. This level will refer clients to a Level 2/3 based on need.
    • To be a Level 1 Qualified Assessor you must complete training for: Prescreen Tool, Participants Rights Packet, Grievance Policy, Safety Planning, CoC ROI and HMIS ROI.
  • Level 2- An agency that completes the Mo BoS CoC Assessment Tools with individuals and families wanting to enter the CES and gain services from your specific agency. To be a Level 2 agency, people receiving services must be an at risk population.
    • To be a Level 2 Qualified Access, you must complete the training for: Prescreen Tool, Participants Rights Packet, Grievance Policy, Safety Planning, CoC ROI, HMIS ROI, Intake Tool and Assessment Tool.
  • Level 3- An agency that completes the BoS CoC Assessment Tools with any eligible households wishing to enter the CES, regardless if the individual is receiving services from the agency. This type of Level will assess individuals from other locations. Contact information for this agency will be shared as an access point for any individual needing services.
    • To be a Qualified Assessor at level 3, you need training for Prescreen Tool, Participant Rights packet, Grievance, Safety Planning, CoC ROI, HMIS ROI, Intake Tool and Assessment Tool.
  • Level 4- A Level 2 or 3 agency that is also performing the following duties: managing the prioritization list in HMIS, conducting data entry in HMIS related to Coordinated Entry, assisting with emergency transfers, and making an effort to refer from the Prioritization List within one business day of an agency requesting a referral. Victim services agencies that are keeping paper assessment packets and assisting the list holder with referrals or emergency transfer plans would be considered a Level 4 access point.
    • To be a Qualified Assessor at level 4, you need training on list management and referrals.
